What does it mean regarding full outfits hiding leg wear? it says on outfits that if you wear it your leg wear cant be seen?

This happens only with dresses and suits. Leg wear will be visible every time you wear t-shirts, shirts etc.

I thought that I had this issue sorted, but when I'm wearing an ''outfit'' or someone else is wearing an ''outfit'' I can't see any difference to my leg wear or theirs, so I don't know how that's supposed to work.

When an 'outfit' is worn, the 'legwear' is hidden by the 'outfit'... only the stats will change by changing the legwear, not the appearance... if, however, you change from an 'outfit' to just a top/shirt, the legwear will become visible again, and will change appearance again...
