Sometimes when I refine an item I get an object in the boxes under my skills list such as the Spanish Fly, how do these work and do I really need them? Sometimes I'll find another clothing item that has higher skill points but then I lose my Spanish Fly if I replace it.

Those icons represent improvements you get by refining your items. Refinements can give you a skill boost or, as i like to call it, MAGIC! lol Skill boosts increase your basic skill and "magic" xD gives you something like "special ability" (for example: first attack, double attack, stun, missile damage boost etc)... It is good to refine as many items as you can, cause they make you stronger...

But if you find an item with a higher skill level that hasn't been refined, what then?

Then you use it and refine it by using diamonds (it costs 19 diams) or wait the next day when you get to refine one time for free
